  • Only complete an E-Consult if you are doing a annual review a medical certificate or if you have been asked to complete one.

Our reception staff are now known as Care Navigators
A care navigator in the NHS is a member of a practice's support staff who helps patients get the right care and support. They are also known as patient advisors or care coordinators.  
  Care navigators help patients by:  
Care navigators work closely with a range of services, including:
  • NHS services 
  • Mental health services 
  • GP's 
  • Social services (council) 
  • Housing associations 
  • Charities 
  • Voluntary groups 
  • Social enterprise organizations    
Care navigators are often the first point of contact for patients, and they can be contacted by phone, face to face, online, or by email.
