About the surgery

If you have been advised that a form is ready for collection, it must be collected from Reception between 12:00pm and 6:15pm, Monday to Friday.

Please ensure you attend during these times. Outside of this period, our Reception team is focused on managing high volumes of patient enquiries, processing urgent requests, and supporting patients with immediate healthcare needs.

We therefore ask that all routine form collections take place during the designated collection hours. Your cooperation helps us maintain a safe and efficient service for all patients.
